Who we are
ADL Home Visit Physiotherapy provides mobile physiotherapy in Adelaide and the Adelaide Hills. This policy explains how we handle personal information, including health information, in line with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
What we collect
When you send a referral through our website, we collect the patient's name, phone number, email address, home address and the reason for referral. If you rate your visit on our website, we collect your star rating, any comment and your name if you choose to give it. Ratings are sent privately to us and are not published on our website. When we provide physiotherapy, we also collect information needed for safe care, such as medical history, treatment notes and funding details (for example NDIS, DVA, Aged Care or WorkCover).
How we collect it
We collect information directly from patients where we can, and from people who refer patients to us, such as family members, carers, doctors, support coordinators and care providers. If you refer someone else, please make sure they have agreed to their details being shared with us.
Why we collect it
We use personal information to contact patients, assess and provide physiotherapy, communicate with the patient's other health providers and funding bodies where needed, process payments and claims, and meet our legal and professional obligations. We do not sell personal information or use it for unrelated marketing.
Who we share it with
We only share information when it is needed to provide care or when the law requires it. This may include the patient's GP or other treating practitioners, funding bodies and insurers involved in the patient's care, and people the patient has asked us to involve.
We use trusted service providers to run our website and email. Referral forms are processed by our website host (Vercel) and delivered to us by email using Google and Zoho services. These providers may store information on servers outside Australia, including in the United States.
Our website shows exercise videos from YouTube (owned by Google) in privacy-enhanced mode. YouTube does not set cookies until you play a video; once you do, YouTube’s own privacy policy applies.
How we protect it
Our website uses encrypted (HTTPS) connections. We take reasonable steps to protect personal information from misuse, loss and unauthorised access, and we keep health records for as long as required by law.
Accessing and correcting your information
You can ask to see the personal information we hold about you, or ask us to correct it, by contacting us using the details below. We will respond within a reasonable time.
